I don't see the point of hacks. You can't take credit for your own actions. There's nothing to brag about if you win something. It ruins both your own and your fellow player's game experience.

In Pokemon, when fighting a gym leader and you put their pokemon on low health. Just when you think you've won they use a full restore.

The Nightmare system, and level design in Mega Man X6.
People who intentionally feed the enemy in League of Legends.
Any RPG that requires a lot of grinding, and/or has a high encounter rate, or doesn't have save points in dungeons.
The camera in any 3D Sonic game.

I hate it when you can't tab out of games.

Or installers with music or full screen ones. Dayum, those get easily on my nerves.

Fallout 3's bugs. All of them. None of them to help you in any way. All of them to annoy the living shit out of you. And even worse granted when Bethesda gave up all hope on the darned thing to work on New Vegas and Skyrim....haaa....hopefully they make it way better in Fallout 4...

New Vegas is pretty much bugfree though. Pretty clear some other company worked on it

I didn't get that sort of vibe when I played Knights of the Old Republic II and Alpha Protocol.

I think its more to do with the Gamebryo engine than the company producing it, which explains the bugs in the Fallout 3 and New Vegas and a few of the Elder Scrolls. It's known for bugs. However if you have a particularly powerful computer you can work around it. Though luck may play a part too.

OT: I hate when playing Dissidia, your friend can just spam HP attacks =__=

Fallout 3 and New Vegas use a different engine than Skyrim. Skyrim runs the Skyrim engine which they'll use for the next Fallout.
